We offer our best service to the people with people with special needs, so that our loved one with disabilities does not feel left out from the society. Together with the family and friends and our dedicated staff and the volunteers we try to the impossible thing possible.

What’s my role?

What to be a part of a community that helps each other to make a favorable environment for everyone? Our Support Coordinator for providing and arranging supports to patients and to supervise the plan and implementation of the orders.

You are required to deal with the patients and their families/ guardians connecting and informing the supports and services that are been provided and arranged for them.

Focus key role:

  • Work in consultation with clients and their families providing and find the services and supports that a patient may need
  • Coordinate services and supports like liaise with government agencies, obtain quotes and organize service agreements
  • Develop, monitor, review and report on your client’s NDIS plans
  • Building and maintaining relationships with our clients, their families, staff, peers, and other services
